What it means in plain English

Your hybrid doesn't have an alternator. The job of keeping the 12V battery charged belongs to the DC/DC converter, which steps the high-voltage system down to 12 volts. P0A94 means that converter isn't performing as expected. The reason this strands cars is simple: once the converter stops charging the 12V battery, the 12V battery runs down, and when it does the car can't stay awake — no READY, no restart. It presents like a dead battery, which is why it gets misdiagnosed as one.

Which Toyota and Lexus models this applies to

Toyota and Lexus hybrids. It comes up notably often on the 2010–2015 Prius, where the converter is part of the inverter assembly.

Possible causes

Plural on purpose. Any of these can produce this result — which is exactly why a code is not a diagnosis.

  • A failing DC/DC converter, which on many models is integrated into the inverter assembly.
  • An inverter fault affecting the converter section.
  • Cooling problems in the inverter loop — low coolant, air in the circuit, or a failed inverter coolant pump.
  • A failing 12V battery loading the converter abnormally.
  • Wiring, connector or ground faults between the converter and the 12V system.

What a professional diagnostic process should check

  1. 1Measure actual 12V system voltage with the vehicle in READY — the converter should be holding the system up, and if it isn't, that's measurable in minutes.
  2. 2Test the 12V battery independently, because a failing 12V battery and a failing converter produce overlapping symptoms.
  3. 3Check inverter coolant level, condition and pump operation, since the converter shares that cooling.
  4. 4Scan all modules and record everything stored — low system voltage sets unrelated-looking codes across the car.
  5. 5Confirm whether the converter is separately serviceable on this model or only as part of the inverter assembly, because that determines the repair entirely.

What you can safely do now

  • Notice the pattern: a hybrid whose 12V battery keeps going flat, or that died with no warning and jump-starts only briefly, fits this fault well.
  • Take the vehicle somewhere rather than replacing the 12V battery a second time — a converter fault will flatten the new one too.
  • Call a Toyota or Lexus dealer with your VIN and ask whether any extended coverage applies to the inverter or converter on your vehicle. Toyota has issued extended coverage on inverter assemblies for some model years, and this is exactly the situation where it's worth ten minutes on the phone before paying anyone — including us.

What not to do

  • Don't keep replacing 12V batteries. If the converter isn't charging, a new 12V battery is a consumable, not a fix.
  • Don't assume the hybrid pack is at fault. This is a converter code, and the pack may be perfectly healthy.
  • Don't set off on a long drive with this stored — the failure mode is the car not restarting.

When towing or urgent service is appropriate

Lean toward towing. The characteristic failure is the vehicle losing 12V supply and refusing to enter READY, and where that happens is not up to you. If the car is running now and you're close to a shop, driving straight there is reasonable — a trip across the Valley is not.

The same code can be set by several different faults, and clearing it without a diagnosis just hides the evidence. P0A94 — common questions

Does P0A94 mean I need a new hybrid battery?
No. It points at the DC/DC converter, which is a different component with a different job. The hybrid pack may be in good condition. This is one of the clearest cases where a proper diagnosis saves someone from buying the wrong expensive part.
My 12V battery keeps dying. Is this why?
It's a strong candidate. The converter is what charges the 12V battery in a hybrid — there's no alternator. If it isn't doing that, the 12V battery discharges over a few days and the car won't wake up. Repeatedly replacing the 12V battery is the usual sign this was missed.
Is the converter covered under any Toyota warranty?
It can be. Toyota has extended coverage on inverter assemblies for certain model years, and the converter is part of that assembly on many vehicles. Whether yours qualifies depends on the VIN, so call a dealer and ask before you pay for the repair. We'd rather tell you that than sell you something you can get covered.
